How it works:

* Qualifying orders will receive redemption form in shipment.

* Redemption form includes instructions directing you online to redemption website www.rewardsofchoice.com/holiday

* The redemption form includes your unique passcode and zip code information allowing entry into website.

* Customers must redeem their gift cards by date on their redemption form in Terms and Conditions.

* Once entered you will select your Gift Card(s) and you will receive it in 2-4 weeks.
